ISD Vaccination Clinics

If your student received the first dose of the COVID-19 vaccine at the Independence School District and City of Independence vaccination clinic at school, they will receive the second dose of the vaccine on June 15th. You will need to complete a second consent form. Please read the information below.

High School and IA:

You need to complete and return this consent form by June 11th. We have printed copies available at each high school if you do not have access to a printer. Students who are 18 can sign their own consent form. Students must also bring their vaccination card. Students will receive their second dose on June 15th at Truman High School or Bingham Middle School. Students not participating in Summer Success should go to Truman High School on June 15th between 8:30 a.m. and 10:00 with a signed consent form.

Middle School:

You need to complete and return this consent form by June 11th. We have printed copies available at each middle school if you do not have access to a printer.  Students must also bring their vaccination card. Students will receive their second dose on June 15th at Bingham Middle School. Students not participating in Summer Success should go to Truman High School on June 15th between 8:30 a.m. and 10:00 with a signed consent form.

What to Wear: The shot goes into your arm just below your shoulder so please wear something that allows you to expose that part of your arm comfortably. And don’t forget your mask!

What to Bring: Students must bring their vaccination card. 

Once students receive the vaccine, they will be monitored for 15 minutes for any reactions and return to class.
