A Residency Affidavit is required when a student is living with an Independence School District resident in order to prove residency within school district boundaries. This document must be renewed annually and cannot be done at your student’s school. Instead - this is done at the annual Residency Affidavit Enrollment Event.

Mondays - Fridays from 8 a.m. to 3:30 p.m. with extended hours on Thursdays (8 a.m. to 6 p.m.)

What to DO BEFORE the event:

Please complete your student’s online registration. If you need assistance, it can be done on-site at the event with the help of an ISD Admissions specialist.

What to BRING TO the event:

You must bring these to the Residency Affidavit Enrollment Event:

1. The district resident with whom the student resides. This person must be present, with their valid photo ID.
2. Printed copy of most recent utility bill of district resident's address, dated within 30 days [gas, electric, or water (Sugar Creek)].
3. Printed copy of district resident's mortgage payment, deed of trust, housing contract -or- copy of rental or lease agreement.
4. Printed copy of parent/guardian's proof of residency (paystub, bank statement, or court documents).
5. Parent/guardian's valid legal photo ID.
6. Residency Affidavit Form English | Spanish.
You may print and complete this form ahead of time and bring it with you -or- you may complete it in person during Residency Affidavit Enrollment Event. This form will be notarized by the ISD at either location.

If you do not come to the in-person event, or your paperwork is incomplete/incorrect, you will have to schedule an Appointment with an Admissions Specialist, and those appointments begin August 18th (the first day of school). The Admissions Team will not see Residency Affidavit families from July 21st - August 15th, even if you go to IA&T in person. This is why we schedule two full weeks to serve returning R.A. families before anyone else, and we send multiple reminder communications leading up to, and throughout, the two-week long enrollment window.

Appointments are required for returning Residency Affidavit families who did not enroll their student during the in-person Residency Affidavit Enrollment Event. The R.A. window closes Friday, July 18th and reopens Monday, August 18th (by scheduling an appointment using the buttons below.)

I missed the Residency Affidavit Enrollment Event: now what?

Because you didn't make it to the in-person Residency Affidavit Enrollment Event, you will have to wait until August 18th to see an Admissions Specialist, at a scheduled-in-advance, in-person appointment. Click here to schedule an appointment.

The ISD begins enrollment each year with our returning Residency Affidavit families - during the two-week long, in-person event. After the event, the Admissions Team must open up their services to the rest of the district and new families. 

This means returning R.A. families will not be seen by the Admissions Team from Monday, July 21st - Friday, August 15th.
